Originating from Northern European lore, he brings good dreams by sprinkling magical sand onto the eyes of children while they sleep at night. The rheum in one's eyes upon waking is believed to be the result of the Sandman's work the previous evening.

Sandmen are the pivotal players in an extra mission called Sandman's backup bag. In the basement of Sector Antlia, the protagonist can find a Sandman who, after explaining that the bag it carries is figuratively its "soul," asks the protagonist to get back his lent bag. After the protagonist encounters a Sandman and asks for it back, it is revealed the other sandman had simply forgotten to return it. Afterwards, a Chakra Pot is rewarded when the protagonist delivers it to the original Sandman.

Sandman appears in the Shinobazu Pond area of Ueno, sometimes accompanied by Oni. It can teach the Dream Needle, Zan and Stun Needle skills during Demon Whisper.
